WAIVER OF LIABILITY, MEDICAL RELEASE, PHOTOGRAPHY/VIDEO RELEASE & INDEMNIFICATION AGREEMENT

This agreement is intended to release and hold harmless Casita Workshop LLC, its owners, instructors, volunteers, and agents from any and all liability, except in cases of sole negligence or intentional misconduct, arising from participation in Casita Workshop LLC activities by myself or my minor child(ren).

By participating, I acknowledge and agree to the following:

​

1. I voluntarily assume all risks associated with participation and agree to release and hold harmless Casita Workshop LLC and Glass Mania Studios LLC, its owners, employees, instructors, volunteers, and agents, except in cases of sole negligence or intentional acts.

​

2. I understand that activities may involve the use of tools, equipment, and materials that could be hazardous, and that participation may result in injury, death, or property damage. Despite these risks, I voluntarily choose to participate or allow my minor child(ren) to participate.

​

3. I agree to be financially responsible for any medical, hospital, or related expenses incurred by myself or my minor child(ren) as a result of participation.

​

4. I grant permission to Casita Workshop LLC to photograph and/or video record me or my minor child(ren) during participation in activities. I understand these images may be used for promotional purposes, including on the Casita Workshop website and social media.

​

5. I certify that I (and/or my minor child(ren)) are physically able to participate in the registered activity. I confirm that I am the legal guardian of any minors I register.

​

6. In the event of an emergency, I authorize Casita Workshop instructors to obtain necessary medical treatment for myself or my minor child(ren).

​

I have read and fully understand this Waiver of Liability. I acknowledge that this is a legally binding agreement and applies to all current and future participation in activities at Casita Workshop LLC.
